Initial Call and Meeting

From the moment the client calls, we begin considering the patient’s specific needs. On the initial call, we ask questions to form a basic needs assessment. Then we set up an in-home visit with the family – including the family member who may ultimately benefit from our care. This visit allows us to gather even more information, answer questions and let your client get a feel for how we could help your family.

Assessment

  The first step in any home care pre assessment is to establish if we will be necessary or beneficial to your loved one. Once you’ve determined that your loved one may require home care, you’ll want to meet with professional care providers. We will perform a complete home care assessment before starting care to determine your loved one’s care needs. This makes it possible to precisely plan your loved one’s care according to his or her specific care concerns. During a home care assessment, we will discuss the patient’s greatest age-related or disability-related challenges, lifestyle and daily schedule, and your overall goals for care.

Placement Offered

If both parties, the family and the company both agreed with the placement offered, a letter will be given as discussed in the meeting to formalize and commence care.

Custom Care Plan

  With Star Home Nursing, you will always know what services are being provided to you. After our initial meeting, we develop a custom care plan. This plan is tailored to a person’s specific needs, and we make detailed recommendations on exactly what kind of services we feel would benefit them. We then arrange a follow-up meeting to go over the plan in detail and modify it as your client sees fit. We follow an established care plan that is composed of Assessment, Diagnosis, Planning, Intervention and Evaluation.

Nurse Matching

Once a plan has been made, we begin our search for the best person to implement that care. Our proprietary system of matching staff nurses to clients takes numerous factors into consideration:
  • Needed service
  • Client’s interest
  • The personalities of both the client and staff
Moreover, if there are any concerns about a specific staff, we can help you search and provide a replacement at a given time.

Care Supervision

Our staff is well-documented about the care they give to you. Our patient relations coordinator and nurse supervisor conduct home supervisory visits to ensure that the needs are met, and the care plan is implemented accordingly. The nurses send updates during and after their shifts.
